PUERPERAL SEPSIS

Diagnostic features:
• This is an infection can occur post-delivery. Organisms that are normally present in the
genitalia of pregnant women for example : E-coli, gonococci/ gardnella vagintis and
streptococci, invade the endometrium and surrounding structure including the lymphatic and
blood stream, thus causing puerperal sepsis.
• The main clinical features are fever, lower abdominal pain and profuse lochia.
• Enlargement of the uterus.
• Peritonitis and pelvic abscess may occur.
• Urinary tract infection occurs.
Investigations :
• Urine (routine)
• Urine (culture/sensitivity)
• Blood test( Hb, TC, DC, ESR)
Treatment / management :
• Give Ampicillin 2g IV every 6 hourly plus Inj Gentamycin 5mg/kg body weight plus Inj
Metronidazole 500mg IV every 8 hourly.
• Give combination of antibiotic until the women is fever free from 48 hours.
• Give Paracetamol 500mg or Ibuprofen 400mg TDS and SOS until pain and fever subsides.
• Maintain personal hygiene.
• Plenty of fluid.
Referral
• Refer patient to hospital for further treatment after immediate care if given.
